2009/2010 Course Catalogue - the catalogue includes a list of forms and instructions for applying for degree programs. If you plan to take courses but are not interested in earning credit please write "AUDIT" on the top of your application.

Application Form - all potential students will need to complete and return this form

Recommendation Form - if you are seeking a degree, recommendations are requested from people who know you well. Make copies and give this form to individuals who agree to complete it for you. They may return the form by mail.

Statement of Prior Experience- complete this form to provide information about your ministerial experience. If life experience credit is desired references will be requested from people who have observed the applicant in ministry.

In addition to the forms included above each student is asked to write a ministerial biography. Your packet includes a page of demographic information. This information is collected for statistical data only. Please fold this page by itself and tape or staple it shut.
